It looks like it happens to territories that go outside the allowed canvas size for maps.
Maps are limited to 3200x2400 pixels. You can upload maps that go beyond that but only territories inside the canvas limit will be clickable.

It seems the parts outside now show fancy skulls.

You're asking the wrong question.

Don't ask why are there skulls, but HOW DO I GET MORE SKULLS?

I have never noticed them before. I guess it takes skulled eyes to perceive them.

check the left side of the dance of the dragons map, you'll see them there too.
