Sometimes when you go to the template of a game you're playing you get presented with two options: Create from game and Create from Template, instead of just getting the second option.
What is the difference between these two? And why is it shown in some games but not all?

You only get it if the template the game was created from (if any) is different from the settings of the existing game.

I think QM does it. The game has a custom message from the host, which is not a part of the template.

Based on my CW games, I dont think the custom message that QM/CW adds really affects it:
