I used to play on the "Multi-attack Split-Mode Commander Cards!" quickmatch template, and it seems it's not available anymore (it looked like that: https://www.warzone.com/MultiPlayer?GameID=26827787).

Has it been removed from the quickmatch templates ? Is it something that happens regularly ?

It was removed because it's below rank 40 in the voting now. The system updates from time to time and if the ranking changes, the new top 40 templates will be playable and those that drop out won't be anymore.
