I noticed that moderately popular templates are falling off the lists (18 and 16 votes). Not on leaderboard, not able to check on template selection to play. Was this planned?

On Leaderboard, I see 33 games that I can select. I am at rank to see 33.

Is it tied to how many you can actually play?

As more and more people get QM votes, the number of votes required to make the top 40 will only go up.

I think it should be programmed so that once you gain access to a template, you keep that access even if it falls below your level.

That is exactly how it is programmed. The only time you lose a template is it falls off Quickmatch completely for everyone.
